I. DAMAGE REPORT :
Anda adalah KKM kapal MT. Trans Pacific yg berbendera Liberia dan didaftarkan di Monrovia. Ketika kapal anda sedang membongkar muatan minyak mentah di Pelabuhan Kobe jepang pd tgl 21 Februari 2005, terjadi kerusakan pada “valve seat ring” berupa goresan sedang, yang menyebabkan level of ballast water menjadi rendah (berada dibawah tingkat normal).

Dear sir
Subject : Damage to the valve seat ring.
I deeply regret to report that the following deficiency and damage to the valve was found and would like to deliver on order of the spare part attached herewith for running repair.
The detail of damage and precaution taken are reported as follow
- Date : 21st February 2008
- Place : Port of Kobe Japan
- Description: Level ballast water of APT lowered in spite of the valve kept closed position
- Investigation : On 21 of February ,the valve was checked after piping disconnection then it was found that the seat ring got turned up with small cut damage
- Treatment : The seat ring was restored in original position then piping connected without replacement due to having no spare part . The test of open and shut were carried out with conformation of airtight
- Counter measure: To prevent air sucking by ballast pump the valve is kept closed position until the seat ring is received.

II. FREE COMPOSITION
Buatlah karangan dlm bhs inggris yg baik dan benar tentang Duties of chief engineer yg meliputi kegiatan-kegiatan KKM dan berdasarkan informasi dibawah ini :
a.General prime duties on board (supervisor, consultant, devision maker)
b.Structural and managerial functions (head of the engine department)
c.Referring to the engine log book (check piston, cylinder, F.O, L.O, engine rotation)
d.Referring to the ship operation and maintenance (plan, discuss and report)
Jawab :
Chief engineer is a head of engine department on board and report to the master. He responsible of operation and maintenance of the whole power units, and he also act as supervisor and consultant.
He also responsible of engine department employers of 1st/e, 2nd/ E, 3rd/e and engine ratings.
Refer to engine log book, he observed the engine rotation, exhaust temperature, fuel oil and lubricating consumption and analyzed the data with previous one to get degree of effectiveness of engine operation.
He responsible to make a plan for routine maintenance and prepare for docking and discussed with sub-ordinate engineer in case of engine trouble.
Last but not least, report to the master for taking unusual step such as to stop or to slow down the main engine rotation.

You should replace the damaged liner ashore as soon as possible. It need for replacement with the new one. In the mean time, I advise you to make the temporary repair. I can meet you when the vessels is alongside in Singapore.
Please advise the agent In Singapore to contact me with the estimated time of arrival.
